SmooveX’s COVID-19 Test Booking Terms and Conditions

  • I agree to the collection, use and/or disclosure of my personal data to SmooveX and its partners to any extent necessary. The disclosure coverage includes processing my orders, as well as receiving offers, promotions, and information about their goods and services. 
  • My medical tests will be conducted, generated, and sent to me directly by Fullerton Health Group. I understand that SmooveX is not liable for any damages or legal responsibilities related to my medical tests and their results. 
  • I understand that SmooveX does not provide cancellations or refunds once my medical test booking is confirmed.

Pre-Departure Testing Checklist:

  • Allocate adequate time to take and receive your test results before departure. In general, travellers who present themselves at the clinic less than 38 hours before the scheduled flight may not receive their test results on time. Test results will be emailed to you within 36 hours after testing. 
  • Double-check the documentation, quarantine and/or testing requirements with the relevant authorities before booking. SmooveX is not liable for any legal responsibilities and/or costs incurred due to any travel rejections by the airlines and/or authorities.
  • Ensure that your pre-departure booking is within the testing window and is in line with your destination country’s requirements. SmooveX is not responsible if you miss your appointment or make the wrong appointment booking based on your destination country’s pre-arrival testing requirement timeline. 

 

My Medical Test and Results:

  • I am fully aware that my medical tests will be conducted, generated, and sent to me directly by Fullerton Health Group. SmooveX will not be liable for any damages or legal responsibilities related to my medical tests and their results. 

 

Terms of Agreement:

  • I agree to the collection, use and/or disclosure of my personal data to SmooveX and its partners to any extent necessary. The disclosure coverage includes processing my orders, as well as receiving offers, promotions, and information about their goods and services.
  • I voluntarily provide my personal data to SmooveX. Should I choose not to submit the required personal data to them, SmooveX won’t be able to contact me nor provide the products or services that I need.
  • If you provide the personal data of anyone other than yourself (including your family members, employees, etc), you warrant that you have informed them of the purposes for which we are collecting their personal data and that they have consented to your disclosure of their personal data to SmooveX for those purposes.
  • Should I provide the personal data of anyone other than myself (including my family members, employees, etc), I warrant that I have informed them of the purposes for data collection, and that they have consented to my disclosure of their personal data to SmooveX.
  • I confirm that I have double-checked my travel requirements before booking my medical tests. Thus, I understand that SmooveX is not liable for any legal responsibilities and/or costs incurred due to travel rejections by the airlines and/or authorities.
  • I confirm that I have double-checked the overseas vaccination requirements before booking my medical tests. Thus, I understand that SmooveX is not liable for any legal responsibilities and/or costs incurred due to the rejection of overseas vaccination verification by the authorities.

Cancellation Policy:

  • I understand that SmooveX does not provide cancellations or refunds once my booking is confirmed.
